Active Sheet Metal Workers

The Sheet Metal Workers’ Health Plan of Southern California, Arizona & Nevada is now conducting its Annual Open Enrollment for eligible Active participants. If you wish to change your medical plan, now is the time to do so.

Eligible Active (Plan A and Plan B) Sheet Metal Workers 

You may have a choice of up to four (4) medical plan options, depending on your residence. Please review plan information very carefully even if you do not wish to change plans, as it includes the most recent benefits and plans available.

IMPORTANT- the Delta PPO plan has been reopened to eligible Plan A participants, only. Please refer to the Enrollment Packet Request form for information on how to enroll in the Delta PPO Plan. 

How Do I Change My Medical Plan?

After you have reviewed the material, if you would like to receive a complete enrollment packet, which includes more detailed information on the Plans available, complete and return the appropriate PDF Packet Request Form to the Administrative Office, no later than November 17, 2025 -OR- to expedite, complete our online request form. Please do NOT return a Packet Request Form if you have requested materials via our online form.

IMPORTANT: Returning a completed Packet Request Form does not change your current plan, or enroll you in a medical plan – it is simply a request to receive detailed enrollment materials, which includes a current directory of the participating providers and facilities.

Upon our receipt of your completed Packet Request Form, we will mail the appropriate materials to you. After you have received and carefully reviewed the information you requested, should you decide to change your medical plan, you must complete, sign and return the appropriate Enrollment Form(s) (included in each plan’s enrollment materials) to the Trust Fund Administrative Office.

The deadline for submitting a completed Medical Plan Enrollment Form to the Administrative Office is December 15, 2025.

The Packet Request Form is NOT an Enrollment Form.

You will receive written confirmation of your enrollment and effective date from the Administrative Office.

What If I Don’t Want To Change My Medical Plan?

If you do not wish to change your medical plan at this time, you do not need to do anything. Remember, you will not have the opportunity to change plans again until the next Annual Open Enrollment period.

The Annual Open Enrollment period is the only time you can change your medical plan selection. Exceptions are made only if you move outside of your selected plan’s service area.
